Is it bulky on a belt?

The body measures 3.54 x 0.79 x 6.69 inches. A pouch for a large phone adds visible width to a belt, especially around doorways, truck seats, or tight work areas. Try the hip, rear belt position, or pack strap to find what stays out of your way.

Does it protect a phone if I fall on it?

The pouch uses nylon and a soft inner lining, and protection. It helps with scratches, bumps, and ordinary carry, but it is not a hard-shell impact case.  

Will the MOLLE attachment sit secure on every rig?

The rear mounting uses webbing and snap loops.  Mount it tightly, check both snaps, and test the fit before a full day outdoors.

Is the flag patch sewn on?

1 removable flag patch is included. It attaches to the front loop panel rather than being sewn into the pouch. Leave the included patch on, replace it, or run the pouch with an empty loop panel.
